Obiettivi formativi

Fornire ai futuri farmacisti conoscenze sui temi delle prevalenti condizioni di multimorbidità della popolazione anziana, le cui patologie croniche richiedono spesso un trattamento polifarmacoterapico. Vengono fornite conoscenze sulle strette relazioni tra polifarmacoterapia e fragilità, in stretta connessione con il modulo integrato di 1 cfu affidato ad un esperto geriatra. Vengono discussi i problemi di appropriatezza prescrittiva nell’anziano, di integrazione di linee guida di prescrizione di farmaci, i rischi prevalenti di interazioni tra farmaci.

Vengono acquisite conoscenze in merito al concetto di aderenza terapeutica, alla sua definizione e ai rischi connessi ad un comportamento non aderente dei pazienti, alle cause più frequenti di comportamenti di non aderenza. Vengono fornite anche competenze in merito agli strumenti informativi, educativi che si possono mettere in atto per supportare un’attitudine aderente alla terapia del paziente. In merito alle modalità con cui l'aderenza può essere valutata a partire da dati sanitari elettronici ci sarà il contributo di un Visiting professor, il prof. Samuel Allemann dell'Univesità di Basilea, esperto nel campo.

Vengono fornite conoscenze in merito ai concetti di patient empowement e di patient engagement.

To provide future pharmacists with knowledge on the prevailing multimorbid conditions of the elderly population, whose chronic pathologies often require polypharmacotherapy treatment. Knowledge is provided on the close relationship between complex pharmacotherapy and frailty, in close connection with the integrated module offered by a frailty expert geriatrician. The problems of prescribing appropriateness in the elderly, the integration of drug prescription guidelines, the prevailing risks of drug-drug interactions are discussed.

Knowledge is acquired regarding the concept of therapeutic adherence, its definition and the risks associated with non-adherent behavior of patients, the more frequent reasons for non-adherent behaviors. Expertise is also provided regarding the information and educational tools that can be implemented to support an adherent attitude to the patient's therapy. Regarding  adherence assessment starting from electronic health data, there will be the contribution from a Visiting Professor, Prof. Samuel Allemann of the University of Basel, an expert in the field.

Knowledge is provided on the concepts of patient empowerment and patient engagement.

CONOSCENZA E CAPACITÀ DI COMPRENSIONE

Gli studenti devono saper riconoscere le criticità che possono emergere in trattamenti polifarmacoterapici, acquisire competenze nell’applicazione dei criteri di appropriatezza prescrittiva nell’anziano (Beers, STOPP&START, ACB, ecc.), in un contesto di ricognizione e riconciliazione terapeutica, devono sapere applicare strumenti per riconoscere condizioni di pre-fragilità o di fragilità connesse a trattamenti polifarmacoterapici.

CAPACITÀ DI APPLICARE CONOSCENZA E COMPRENSIONE
Gli studenti sono messi in condizione di applicare le competenze acquisite, di utilizzare gli strumenti disponibili per l' analisi dell'appropriatezza di polifarmarmacoterapie ragionando su casi reali e di comprendere quindi i rischi conseguenti per il paziente di peggiorare la propria condizione di fragilità. 

AUTONOMIA DI GIUDIZIO
Gli studenti sviluppano la capacità di analizzare autonomamente regimi polifarmacoterpaici per individuarne le criticità e identificare le soluzioni

ABILITÀ COMUNICATIVE
Il corso permette agli studenti di sviluppare abilità comunicative nel fornire indicazioni sull'uso corretto dei farmaci, sia nella relazione con i pazienti, sia nella relazione con le altre figure di professionisti sanitari. L'obiettivo è di preparare figure professionali consapevoli e proattive nei confronti delle problematiche affrontate dal corso.

CAPACITÀ DI APPRENDIMENTO
Gli studenti sono stimolati ad apprendere l'uso di strumenti per riconoscere sia condizioni di pre-fragilità o di fragilità conseguenti a trattamenti polifarmacoterapici, sia le problematiche conseguenti a prescrizioni inappropriate o al comportamento non aderente dei pazienti. 

KNOWLEDGE AND UNDERSTANDING

Students learn to recognize the critical issues that can emerge in polypharmacotherapy treatments, acquire skills in the application of prescribing appropriateness criteria in the elderly (Beers, STOPP&START, ACB, etc.), in a context of therapeutic recognition and reconciliation, they must know how to apply tools to recognize pre-frailty or frailty conditions connected to polypharmacotherapy treatments.

APPLYING KNOWLEDGE AND UNDERSTANDING
Students are put in a position to apply the skills acquired, to use the tools available for the analysis of the appropriateness of polypharmacotherapies by reasoning on real cases and therefore to understand the consequent risks for the patients of worsening their frailty condition.

INDEPENDENT JUDGEMENT
Students develop the ability to autonomously analyze polypharmacotherapy regimens to identify critical issues and identify solutions

COMMUNICATION SKILLS
The course allows students to develop communication skills in providing indications on the correct use of medicines, both in the relationship with patients and in the relationship with other health professionals. The aim is to prepare professional figures who are aware and proactive towards the problems faced by the course.

LEARNING SKILLS
Students are encouraged to learn the use of tools to recognize both pre-frailty or frailty conditions resulting from multi-pharmacotherapy treatments, and the problems resulting from inappropriate prescriptions or non-adherent patient behavior.

Definizione di fragilità secondo i modelli biomedici legati al concetto di decadimento fisico o di sindrome conseguente a fattori negativi che incrementano la vulnerabilità dell’individuo (modelli di Fried e di Rockwood)

Definizione di fragilità secondo il paradigma bio-psico-sociale, proposto da Gobbens, studio dello strumento Tilburg Frailty Indicator (TFI).

Le relazioni esistenti tra le condizioni di fragilità o pre-fragilità dell’anziano e i trattamenti polifarmacoterapici che riguardano i pazienti anziani affetti da multimorbidità croniche.

La polifarmacoterapia e i rischi di non appropriatezza prescrittiva nell’anziano (crieteri espliciti e impliciti di non appropriatezza), di reazioni avverse da interazioni tra farmaci, di ageismo, indicazioni alla deprescrizione.

Supporti ICT al processo di ricognizione e riconciliazione terapeutica.

Definition of frailty according to biomedical models linked to the concept of physical decay or syndrome resulting from negative factors that increase the individual's vulnerability (Fried and Rockwood models)

Modalità di insegnamento

L'insegnamento si svolge in presenza secondo l'orario previsto, che viene riportato anche in fondo a questa pagina.

La frequenza è caldamente consigliatata in quanto è prevista una continua interazione tra studenti e docenti (anche invitati esterni) e tra studenti, in quanto è previsto che, fin dalle prime lezioni, gli studenti comincino a lavorare in coppia (o se necessario in gruppi da 3) alla preparazione di un elaborato che costituirà, alla fine del corso, il materiale per la verifica dell'apprendimento.

Il corso è pertanto fortemente improntato ad una metodologia "learning by doing" in quanto per ogni argomento teorico affrontato è previsto un immediato riscontro di tipo pratico che consente di applicare e apprendere velocemente i contenuti.

Tutto il materiale necessario alla preparazione del corso è disponibile sulla piattaforma di Moodle: oltre alle tradizionali diapositive, saranno disponibili le registrazioni delle lezioni in streaming degli anni accademici precedenti, materiali didattici di approfondimento, materiale integrativo con link a siti web, filmati.

Sono previsti interventi seminariali tenuti da esperti nei settori dell'ICT applicata alla ricognizione e riconciliazione terapeutica, da esperti nel campo del patient engagement.

The course takes place in presence according to the scheduled timetable, which is also reported at the bottom of this page.

Attendance is strongly recommended as there is a continuous interaction between students and teachers (even external guests) and between students, as it is expected that, right from the very first lessons, students start working in pairs (or if necessary in groups of 3) for the preparation of a project which will constitute, at the end of the course, the basis for learning assessment.

The course is therefore strongly based on a "learning by doing" methodology since for each theoretical topic addressed, there is immediate practical application that allows rapid learning of the contents.

All the material needed to prepare for the course is available on the Moodle platform: in addition to the traditional slides, streaming recordings of lessons from previous academic years, in-depth teaching materials, supplementary material with links to websites, videos will be available.

Seminars will be held by experts in the fields of ICT applied to therapeutic recognition and reconciliation, by experts in the field of patient engagement.

Modalità di verifica dell'apprendimento


La verifica dell'apprendimento è condotta attraverso la redazione di un elaborato all'elaborato scritto e successivamente, nella data dell'appello, attraverso una relazione orale con presentazione powerpoint relativa . I contenuti dell'elaborato vengono progressivamente preparati dagli studenti durante il corso dietro la guida della docente.

The learning assessment is conduced through the drafting of a written production and subsequently, on the date of the exame, through an oral report with the relative slides presentation. The contents of the written production are progressively prepared by the students during the course under the guidance of the teacher.
